CHORIOCARCINOME VAGINAL PRIMITIF : UNE TUMEUR RARE HÉMORRAGIQUE

RESUME:
La localisation vaginale d’un choriocarcinome est rare. Nous rapportons le cas d’une femme malgache de 19 ans, ayant eu, 7 mois auparavant, un antécédent de môle hydatiforme et qui a présenté une tumeur vaginale hémorragique. L’examen histologique a permis de poser le diagnostic de choriocarcinome. Faute d’immunohistochimie, un dosage sérique de bHCG a été demandé pour confirmer le diagnostic. Notre observation souligne l’intérêt d’une surveillance correcte et régulière des patientes ayant présenté des môles hydatiformes, afin de détecter à temps la survenue d’une complication comme le choriocarcinome. Devant la présence d’une tumeur vaginale hémorragique chez une femme jeune avec un antécédent de môle hydatiforme, il faut penser à l’éventualité d’un choriocarcinome et demander un dosage de β-HCG.

SUMMARY:
Choriocarcinoma primitive is an uncommon tumor rarely seen in the vagina. We report a case of a 19 years-old Malagasy woman, with an antecedent of hydatidiform mole, who presented a vaginal choriocarcinoma. It was diagnosed by histological examination associated with a high blood level of βHCG. Toward hydatidiform mole, adequate medical follow-up is necessary to avoid complication such as choriocarcinoma. The possibility of a choriocarcinoma must be evoked in the presence of bleeding vaginal tumors associated with an antecedent of hydatidiform mole. A determination of the blood level of βHCG is required to confirm the diagnosis.
